• Relationship building: clients, families, support workers and external providers all rely on you
• Coordinating support services shaped around clients' goals and their health conditions
• Assessing clients, preparing care plans and reviewing them
• Weighing every decision against client safety, wellbeing and quality of life
• Leading and supervising a Support Worker team for quality and continuity of service
• Monitoring and managing client budgets
• Keeping work compliant with legislation, funding guidelines and internal policies

• Proven aged care, home care, community services or health setting experience
• Sound assessment, care planning and case management capability
• Confidence working without close supervision while managing competing priorities
• Excellent communicator with strong relationship-building and leadership skills
• Working knowledge of My Aged Care
• Working knowledge of the Aged Care Quality Standards
• Driver's licence and own vehicle, with willingness to travel across Sydney
• Bilingual or multilingual ability (a strong advantage)
• A clinical background in nursing, allied health or similar is preferred but not required
